This report addresses the question of how school-based health centers (SBHCs) fit into the patient-centered medical home (PCMH) model of care. It describes what SBHCs across California are doing with respect to PCMH recognition, reviews best-practices that help SBHCs move closer to the PCMH model, and offers recommendations for SBHCs looking to strengthen their PCMH practices.
